- John AlbericoPresident and Chief Operating Officer
John leads the WaveForm Diabetes team with his manufacturing and operational expertise as President and COO. Previously, he served for 15 years as CEO of AgaMatrix, Inc, bringing world-class glucose monitoring devices to market for strategic partners like Sanofi, CVS, Kroger, and One Drop, as well as the AgaMatrix brand. Prior to AgaMatrix, John was the Managing Director of the Inverness Medical Limited (“Inverness”) subsidiary in Scotland. During his 21-month tenure, he spearheaded efforts to increase the operation’s efficiency to cost-effectively produce over 1 billion test strips annually and lead the transfer of the company’s meter manufacturing operations to China.
Prior to Inverness, John was the Vice President of Operations for Bionostics Inc., a medical reagents and controls company, and the Vice President of Manufacturing for Nova Biomedical, a medical instrumentation company. John is also the founder of Medical Devices, Inc., an engineering consulting company specializing in the offshore contract manufacturing of high-volume consumer medical devices.
- Jackson McAdamChief Financial Officer
Jackson is a key participant in WaveForm’s strategic efforts and was integral in the acquisition of the CGM platform from iSense CGM, Inc. and Bayer Healthcare in early 2016. In addition, Jackson has lead financing and tax reorganization efforts including a recent deal with Prospect Capital.
Prior to joining WaveForm Diabetes, Jackson spent a total of nine years with Kronos Incorporated, a private equity owned software and service organization reporting $1 billion in annual revenues, where he held various senior finance positions. Jackson also served as Director of Internal Audit with a supply chain management services company, ModusLink Global, for three years and began his career with PricewaterhouseCoopers.
- Chuck JohnsonVice President of Engineering
A veteran technical manager, Chuck has led engineering development activities for WaveForm, including the development of their current and future continuous glucose monitoring products. Chuck has been a key contributor to WaveForm Diabetes’ product portfolio from the first FDA-cleared blood glucose monitor to connect directly to the iPhone, iBGStar, to their Bluetooth-enabled meter Jazz Wireless 2.
Chuck’s nearly 40 years of engineering experience has involved a number of successful roles in the medical device industry, including Director of Engineering at Eleme Medical (acquired by Cynosure) where he oversaw the development of a laser diode system to reduce cellulite by liquefying fat cells. He was also Vice President, Development Engineering at Candela Corporation where he was responsible for systems to treat kidney stones, vascular lesions, wrinkles, cellulite, tattoo removal, and the removal of unwanted hair. Chuck also managed the development of the company’s variable pulse duration YAG Laser for hair removal, which was the company’s most successful product.

Dave holds extensive experience in new product development and licensure within the U.S., Canada, European Union, Brazil, India and China, especially in relation to IVD, medical devices, biotechnology, and biologics. Prior to joining WaveForm Diabetes, Dave served as Director of Regulatory Affairs, Clinical Affairs and Quality Assurance at Harvard Apparatus Regenerative Technology, Inc., a Holliston, Massachusetts biotechnology company specializing in the development of cell-based tracheal prostatic.
Among his previous roles, Dave served as Vice President, Corporate Regulatory and Quality for five years at SeraCare Life Sciences, Inc., a $60 million IVD biotechnology company specializing in the manufacture of serological and nucleic acid controls and reference materials. Dave has also held executive and management positions with Defibtech, LLC, American Red Cross, and C.R. Bard, Inc.
- Mihailo V Rebec, PhDChief Technology Officer and Head of CGM Clinical Affairs
Dr. Rebec has been intimately involved in the development of pioneering sensor technology used in the WaveForm CGM, dating back to 2004. He is an author on 30+ patents covering instrumentation, algorithm development, and innovative analysis methods. Dr. Rebec continues to lead the technical, clinical, and regulatory efforts for the WaveForm CGM in the EU, US, and China, including directing WaveForm’s CGM sensor research and development work.
Prior to WaveForm’s acquisition of the technology in 2016, Dr. Rebec served as the lead research scientist in the advanced technology group of the Bayer Diabetes Group. He was involved in extensive research in the areas of noninvasive and minimally invasive glucose analysis.
- Cara MottoloVice President of Human Resources
Cara oversees all personnel activities for employees located in the United States, Hong Kong, China and the United Kingdom. A key participant in the company’s acquisitions, Cara has guided the personnel integration for WaveForm Technologies in Wilsonville, Oregon. She also serves as the main point of contact for shareholders and employees regarding equity administration.
During Cara’s 17 years in the human resources profession, she has led efforts in a variety of industries from the medical device company NMT Medical, Inc. to the sports and entertainment venue the Boston Garden (at the time, the FleetCenter). Earlier in her career, Cara spent time in broadcast journalism, holding on-air positions at several local radio stations.
- Deak KimVice President of Operations
Deak is Vice President of Operations for WaveForm and General Manager of WaveForm Diabetes’ Hong Kong-based subsidiary. Deak has been part of the WaveForm Diabetes team since 2006, primarily leading the company’s subsidiary operations in Asia. In addition to his subsidiary management responsibilities, he has been recently responsible for WaveForm sensor manufacturing operations in Wilsonville, OR.
